Homebound Vaccine Program

Flu & COVID Boosters are available to Hampshire County residents who are unable to get vaccinated at a clinic. Please visit this link https://docs.google.com/forms/d/e/1FAIpQLSd5xed0Y4k7tu2W-gWYiLloF_MX0bWYSPJCHT0BNpC45uA1sA/viewform to recommend someone for a homebound vaccine visit and a nurse will follow up with the patient directly. Appointments are typically scheduled within a week.
